Clinical Support Services, Inc. (CSS)s newest program to help Medicare plans monitor and track medication overutilization by Medicare beneficiaries is the latest in a long line of medication management software and solutions.
CSS software is used nationwide in variety of settings including managed care plans, community pharmacies, colleges/schools of pharmacy and independent clinical pharmacy practices. At the heart of CSS is Medication Pathfinder, which provides pharmacists and other practitioners with a web-based program to review a patients medications, medical claims and clinical data in order to determine the most appropriate therapy. MTMPathTM, the software integrated into this program, allows for the development of a patient profile from which potential therapy-enhancing recommendations can be pulled. It was built in S-O-A-P (Subjective-Objective Assessment Plan) Note structure to help pharmacists move more quickly through complex patient Medication Therapy Management (MTM) assessment procedures.
CSSs web-based software serves as a prism. With a robust registry at its core, CSS has developed multiple interface products that are imbedded with the most current evidence-based clinical algorithms to accommodate the unique perspective of the user. For example, an ambulatory pharmacist providing wellness coaching would use the MTMPath software to access the registry, an interface that is very different than the one required by a primary care physician (EHRPath). How the data elements are configured, managed and reported depends on the users clinical requirements.

Ambulatory Pharmacy Model: Medication Pathfinder, CSSs network of credentialed pharmacists, supports MTM efforts using CSSs web-based MTMPath software. CSS has experience (1) creating networks in geographical regions; (2) converting a network from another third-party vendor to CSS; and (3) integrating new programs into its established pharmacy network.
Licensing Model: MTMPath may be licensed to allow a clients clinical team to support and manage MTM efforts. MTMPath provides a clients clinical team with a streamlined process to manage comprehensive medical reviews (CMRs) and targeted medication reviews (TMRs). Created in a S-O-A-P Note format, it accommodates both speed and clinical accuracy.
