As of the end of 2012, health insurance issuers and group health plans are required to provide employees with an easy-to-understand summary about a health plans benefits and coverage an SBC. An SBC is a document detailing, in plain language, simple and consistent information about health plan benefits and coverage. It summarizes the key features of the plan or coverage, such as the covered benefits, cost-sharing provisions, and coverage limitations and exceptions.

All employers offering a group health plan and health insurance issuers (carriers) are required to provide an SBC. An SBC may be printed out and distributed to employees or may be displayed electronically as part of an online benefit website/portal. The federal law requires that an SBC posted online meet the following requirements:
1. The format is readily accessible (such as a PDF document)
2. The SBC is provided in paper form free of charge upon request
3. The provider timely advises that the SBC is available online and gives the internet address (providers may make this disclosure by email).
